Dawn A. Hernandez, 56, Johnstown, died November 29, 2024 at Maple Heights Health and Rehab. Born August 13, 1968, daughter of Gerald and Jeana (Hill) Boonstra. Preceded in death by father. Survived by mother; daughters, Brianna; and Madison; grandchildren, Logan, Liam, and Audreianna Hernandez-DeFazio and brothers, Gavin Boonstra and Matthew Boonstra. By request, there will be no viewing. A memorial service will be held at a later date. John Henderson Company Funeral Home is serving the family.
